We wanted the player to be able make changes to the world, and for their actions that have an impact on how things are going in The world that is Sanctuary. If you find these strongholds you will find a variety of them in the different regions.

There will be others that are overrun by creatures. By clearing them out then you'll be able to make them a secure location for Sanctuary. There will be non-player characters and you can unlock side quests. Other dungeons could be unveiled as a result as well as the strongholds be transformed because of the actions you've taken on that character throughout.

When you win that fight, the area transforms into one that is a social space. Then the same as Joe was just saying it could be a town, or it could be a dungeon, it could be a new waypoint where you could teleport, but the other players are buy D2R Ladder Items able to be there too.
